Pre-authorization to be seen or treated at the Institute is the patient's responsibility. Patients are responsible for knowing what their plan will cover. If you have any questions please contact the Institute or your insurance company.

When is my co-payment due?
If your medical coverage requires you to make a co-payment, it is due at the time of your visit.

Will the Institute bill my insurance company directly?
Yes. If we have not received payment within 30 days of submitting your claim to your insurance company, you are responsible for payment of the balance due. If you are unable to pay the bill in full at that time, we ask that you contact our Patient Accounting Department at (952) 814-6600 to discuss a monthly payment program. We will then ask that you sign a statement which specifies the terms for payment. If we receive payment from your insurance company after you have paid your account, we will refund that amount to you.

Does the Institute accept credit cards?
The Institute accepts MasterCard, Visa, American Express, & Discover.

What if I don't have medical coverage?
In the event that no medical coverage is available, your payment for services to be rendered is due and payable when you arrive at the clinic.
